Dr Gutierrez only sees insured patients with in-network insurance. Thank you for understanding.

Please register and schedule through the link at the bottom of the page. Then immediately send a message in the portal with subject "Insurance and ID" with photos of the front and back of your insurance card and government-issued photo id to complete your registration.

Please do not register unless you meet the above criteria. Register only when you are ready to submit ACTIVE insurance and set a date for your first appointment. Your file will be deleted in three days if we do not receive your insurance information and ID, AND have your new patient appointment scheduled. 

Example, if your insurance becomes active on January 1, please do not register until January 1 as it will show up as INELIGIBLE until the ACTIVE DATE.

It is the patient's responsibility to understand his/her own health plan, and to verify with their insurance that an office visit to Dr Gutierrez is covered.  The patient will be fully responsible for charges that insurance does not cover.

Your first appointment is a New Patient visit to establish care. It will not be billed as your annual physical. Dr Gutierrez will get to know you and your history, order lab work, and schedule your SECOND appointment as your annual preventive care visit. Your second visit cannot be your physical if treatment is necessary based on lab results or other factors. He asks that all patients schedule a preventive care visit each year.

Dr Gutierrez does not handle Worker's Comp claims.

MVA claims are processed only for previously established patients.

A note to our non-binary patients: We have contacted our vendor about the restrictive M/F gender selection when signing up for the Patient Portal. The issue is under review and complicated by the fact that all 50 states do not recognize non-binary options, yet insurance billing happens across states. Be assured that the Medical History Questionnaire which populates for New Patient and Annual Well check appointments provides non-binary options that become part of your medical record.
